St. Thomas Camp Permission Form and Release

St. Thomas Camp Photograph and Video Consent

Pictures and videos may be taken of your child during his/her time at camp. We would like to be able to use these photographs and videos for flyers, Diocesan publications, and the Diocesan/Camp website and social media accounts. Written consent of the parent/guardian is required. Names will not be posted unless written authorization is given by the parent/guardian, and then only first names will be used. If there are concerns about pictures or videos posted on the website, please contact the Camp Manager and they will promptly be removed.

I/We, the parent(s)/guardian(s) of this youth authorize and give full consent, without limitation or reservation, to St. Thomas Camp/Diocese of Great Fall-Billings, to publish any photograph or video which the above-named camper appears while participating in any activity with St. Thomas Camp. There will be no compensation for use any photograph or video at the time of publication or in the future.
